Patient Care Manager – Philip Hulitar Inpatient Unit

Under the direction of the VP of Patient Care Services, the Patient Care Manager of the Inpatient Unit is responsible for the overall care delivered by the Inpatient Unit staff.  The position maintains close working relationships with IDT managers to ensure patient care services are safe, effective, demonstrate a patient centered coordinated plan of care, and responsive to patient and family needs 24/7 and to fulfill and exemplify the HHCRI mission.  The Patient Care Manager develops, administers, and oversees all aspects of the Inpatient Unit and ensures the provision of high quality services to patients, families, care-givers and outreach to the community.

Clinical Services

Insures that the day-to-day activities and patient care services of the Hulitar interdisciplinary team (IDT) of hospice care professionals are provided in conjunction with other HHCRI teams and services.  Responsible for safe effective resource management – human, fiscal, and material.

Functions with demonstrated knowledge and skills necessary to provide care to patients throughout the life span, with consideration of the aging processes, human development stage and cultural patterns in each step of the care process.

Insures appropriate orientation of Inpatient Unit staff and identifies opportunities for staff to develop new competencies.  Insures maintenance of required competencies and credentialing of Inpatient Unit staff.

Provides direct clinical supervision and evaluation to Nursing and Administrative staff, and indirect supervision of Inpatient Unit assigned Bereavement/Volunteer/Spiritual Coordinators, Nursing Assistants, Social Workers, RNs, Pharmacist and Medical Director.  Counsels and coaches RN’s and other team members (when appropriate) regarding customer focused services and job responsibilities. 

Additionally, manages the unit’s medication delivery system, and acts as Administrator on Call (AOC) for HHCRI on a rotating basis.

Performance Improvement/Regulatory

Identifies potential risk management concerns and communicates information to appropriate others.  Works closely with Quality & Compliance Manager and appropriate others to insure compliance.   Establishes and maintains positive relationships with all customers. Handles concerns/complaints effectively and per agency time frames.

To perform this job successfully, an individual must be a Registered Nurse with Bachelor’s Degree in Nursing; demonstrate evidence of applied knowledge in all state and federal insurance standards, regulations and accreditation standards; possess five years clinical experience with in-depth clinical knowledge to include a minimum of two years nursing experience in a Home Care/Hospice setting and progressive management experience/success; and demonstrate proven leadership ability and highly effective organizational skills.

Sr. Philanthropy Associate

The Senior Philanthropy Associate at Home & Hospice Care of Rhode Island (HHCRI) is responsible for managing a portfolio of assigned individual gift prospects. Under the direction of the VP of Philanthropy, this position is responsible for cultivating, soliciting and stewarding a “circle of caring gift prospects” by fundraising directly with HHCRI prospects, vendors and potential supporters of the organization.

The Senior Philanthropy Associate possesses excellent written and verbal communication skills, the ability to manage confidential information and utilizes a collaborative team approach to develop strategies to obtain major gifts for HHCRI. Will be tasked with the development and implementation of solicitation strategies for gift prospects, as well as strategizing for and implementation of future capital campaigns.

The successful candidate has proven experience in the successful cultivation, solicitation and closing of gifts and possess a working knowledge of the Rhode Island philanthropic market, as well as the Rhode Island profit and non-profit community business community.

Integral to the success of this position is the ability to empathize and communicate with the grieving community of HHCRI’s donor base, and maintaining donor and constituent confidentiality at all times.  Will assist in the coordination and management of HHCRI philanthropic special events. Previous experience running capital campaigns is preferred.  Must have comprehensive knowledge of palliative and end-of-life care

A Bachelors Degree is required. Master’s Degree is preferred with a minimum of three years fundraising experience and the ability to work in relationship building and a broad understanding of gift management and planned giving tools is required. This is a highly independent and multi-faceted role which will occasionally require work outside established business hours, including early mornings, evenings, and weekends as dictated by business needs. Prior healthcare experience is preferred. A comprehensive knowledge of the process of donor movement is necessary. Planned giving and moves management experience a plus.

Registered Nurse Admissions

 Full Time, Monday-Friday, 1pm-9:30pm

The Admissions RN welcomes and admits patients and families to HHCRI care, providing skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing, accrediting bodies, and the policies of Home & Hospice Care of Rhode Island through a competent, compassionate, timely and responsive admission process.

As a RN practicing in the field, the Admissions RN functions as first face-to-face HHCRI contact for patients, families/ caregivers.  Provides assessment, consultation, support and education to patients, families, caregivers and others in the setting in which hospice care will be provided.

This role is responsible for the completion of the patient admission process which includes: review of admission packet with patient and family/caregiver/health care proxy; explain and clarify hospice philosophy; benefits to be expected through HHCRI; how to contract HHCRI, liability for services (if applicable); inform patient of rights and responsibilities; and have patient or health care proxy sign consent forms.

In performing the initial assessment of patient and family, determines the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living and determine appropriateness for HHCRI service.

The Admissions RN uses an interdisciplinary approach in developing, plan of care, in accordance with the organization’s policies.  Initiates referrals to other disciplines and refers to community resources only when necessary to meet an immediate need.

Bachelor of Science Degree in Nursing preferred; two years’ related experience or training; or related equivalent combination of education and experience.  Home care, long term care, admissions and/or hospice experience preferred. A Rhode Island license in Nursing is required along with a valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order. Critical thinking skills are inherent to the position.  The ideal candidate will possess strength in their ability to evaluate situations according to established standards and policies.  In addition, ability to define problems collects data, establish facts and draw valid conclusions.

Medical Records Assistant 

Part-time, 12:30pm-4:30pm, Monday-Friday

The Health Information Assistant performs the filing of all patient documentation accurately and consistently in accordance with established productivity standards, policies and procedures applicable to law and regulations.

Under the direction of the Health Information Services Manager, this position supports and promotes integrity and security of HHCRI patient medical records through meticulous processing of documentation.  Perform duties related to the timely maintenance and organization of patient medical records, including filing and retrieval of health information per department protocols.  Supports clinical documentation quality and review processes. 

Informs and communicates immediately any inconsistencies or variances of the medical records productivity standards, policies, procedures, legal, regulatory and accreditation requirements, timely submission of documentation. Maintains confidentiality at all times. 

Basic computer operating system and software skills including Windows, Explorer, Word, Excel and Outlook.  Ability to work independently with direction on priorities and goals. Well-organized with appropriate time management skills.

High School graduate or equivalent Required.  Knowledge of medical terminology and experience in related healthcare strongly preferred.  Must have a comprehensive understanding of release of information process, including state, federal, and HIPAA regulations.  Demonstrated experience in performing high quality work with meticulous attention to detail with minimal errors.

Registered Nurse – Philip Hulitar Inpatient Unit 

Part Time, 24 hr eve/night 7pm-7:30am w/rotating weekend and holiday coverage  

The Registered Nurse (RN) provides skilled professional nursing care to the patient/ family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Home & Hospice Care of Rhode Island, and under the direction of the Patient Care Manager for the Inpatient Unit.

In a general inpatient setting, provides professional nursing care by coordinating all aspects of patient care from initial assessment through discharge.  Assesses and evaluates patient’s status by (1) Developing and initiating plan of care in collaboration with patient/caregiver/primary health professional, (2) Regularly re-evaluating patient/family/caregiver needs and responding to changes as needed. (3) Conducting on-going evaluation and revision of care according to patient outcomes.

The Inpatient RN facilitates the patient’s care with regard to the activities of daily living by promoting self-sufficiency and optimal comfort care when appropriate.  On a regular basis, will coordinate patient services with other team members to provide continuity of care.  Works in cooperation with the family/caregiver and interdisciplinary team members to meet the identified needs of the patient/family/caregiver; including psychosocial, spiritual and emotional needs.  Uses interdisciplinary approach in developing, supervising and coordinating plan of care, in accordance with the organization’s policies.  Initiates referrals to other disciplines and community resources, as indicated and is necessary.

Bachelor of Science Degree in Nursing preferred; two years of related experience or training; or related equivalent combination of education and experience. Home care, long-term care, oncology, medical/surgical and/or hospice experience preferred. Current Rhode Island RN License with at least two years’ related RN experience. Home care and/or hospice experience preferred.

Director of Finance

Reporting to the CFO, the Director of Finance will oversee financial operations functions including accounting/general finance, financial analysis and reporting, accounts payable, and payroll.   This highly visible position is an integral part of the organization’s leadership team working in close cooperation with the revenue cycle management, decision support, human resources, and other critical functions of the organization.  The successful candidate will have a sincere commitment to the mission of HHCRI, extensive fiscal management and analytical skills, strong written/oral communication and analytical skills, and progressively responsible experience in a financial leadership position preferably in the health care industry. 

Key areas of responsibility for this position include the following:  (1) Oversee & manage accounting, accounts payable, payroll, and cash management functions; (2) Maintain accounting & financial reporting systems, while safeguarding organizational assets; (3) Manage various audits and oversee the agency’s insurance program; (4) Support budget development & management, contracting, decision support & revenue cycle functions; (4)Serve as a key organizational resource and technical expert in financial matters; (5) Advance HHCRI’s mission and vision.

This position requires an in-depth knowledge of financial reporting and analysis including current and changing generally accepted accounting principles and relevant audit standards as they apply to the organization’s financial statements.  Knowledge of and experience with accounting, accounts payable, and payroll processes including relevant laws, guidelines, regulations, and standards, particularly as they relate to health care organizations, not for profits, and hospice providers specifically.

Bachelor’s Degree in Accounting, Finance, or other related discipline with Master’s Degree strongly preferred.  Advanced degree in accounting or finance required with CPA or equivalent certification/experience strongly preferred.  Minimum three to five years progressively-responsible financial management and fiscal operations experience in healthcare setting with supervisory experience.  Experience with insurance, tax-exempt financing, cash management, internal controls, and grant management/reporting a plus.

Registered Nurse

 Part Time, 16 hour weekends

The Registered Nurse (RN) provides skilled professional nursing care to the patient/family/ caregiver as prescribed by the physician and in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing, accrediting bodies, and the policies of Home & Hospice Care of Rhode Island. 

As an RN practicing in the field, is responsible for the initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living. Will support the individualized plan of care and recommend revisions to the plan as necessary including plans for discharge/transfer as appropriate. This position involves ongoing consulting with and educating the patient/family/caregiver regarding the disease process, symptom management/pain management, end-of-life care, and the process for dealing with issues of ethical concern.

The hospice RN works in concert with other interdisciplinary team members, assessing home environment and teaching patient/family/caregiver regarding safety issues.  In addition to providing clinical care, the RN also works in close cooperation with the family/caregiver and interdisciplinary team members to meet the identified needs of the patient/family/caregiver including psychosocial, spiritual, and emotional needs and refers to appropriate disciplines/services.

Registered Nurse – Referral Center 

Part Time, Every Other Weekend Sat, Sun 8:30-5

 

As an office-based RN, the Referral Center Nurse ensures a smooth transition for patients being admitted to Home & Hospice Care of Rhode Island. Provides timely, responsive customer focused and customer friendly service to all referral sources, patients, families, staff, students, vendors and visitors. Acts as a liaison for incoming calls to coordinate visits with the Admissions Nurses and Nurse Liaisons. 

The Referral Center RN acts a customer service liaison between referral sources, care providers, professional staff, patients and families, insurers, physicians and members of the IDG to ensure the smooth and timely admission of patients. Through phone and internet communications, establishes and maintains a positive relationship with all internal and external customers.

Schedules visits for all inquiry and referral callers within required timeframe.  Arranges visits for liaisons and admissions nurses, providing follow up on pending patients and non-admissions.  Communicates prospective patient updates to appropriate referral sources.

On a regular basis, communicates with referral sources, DME, pharmacy, and other vendors to process all certifications, orders, and required admission information. Helps resolve referral and admissions scheduling issues and customer complaints through direct communication with the referral sources, admissions team, Help Center & Admissions Manager and other managers.

Bachelor of Science Degree in nursing preferred; two years of related experience or training; or related equivalent combination of education and experience. Home care and/or hospice experience preferred.

Current Rhode Island RN License with at least two years’ related RN experience. Home care and/or hospice experience preferred.
